This form will generate a link and QR Code for others to complete the Safety Culture Assessment. Fill in the details, submit this form and you will receive an email with a shareable link.
- Anyone using that link / QR Code will be taken directly to the Safety Culture Index survey which will also have some of the information you have entered such as type and stage of production (e.g. TV: Scripted Drama, Prep and Shoot, Post), location (e.g. Studio) and duration pre-populated in hidden fields for analysis later.
- No information that could identify a production (Production Name, Production Company and/or Commissioner) will be included, and the link is fully tokenised for added security.

These will be anonymous in that there will be no information that directly identifies the user who completed the survey in the email you receive, however the email will contain a unique, randomised code so you can identify surveys and productions you have originated, but the code will be meaningless to anybody else.
