Safety: The Survey

The Survey has two headline metrics:

  1. The Safety Culture Index (SCI): how safely is a production / event actually operating in practice, based on real, measurable evidence
  2. The Safety Perception Index (SPI): how do people feel about safety compliance: do they have confidence and trust in safety processes

If the SCI is low, but the SPI is high, people are confident in how safety is being managed – but that confidence is not backed up by the evidence. Conversely, if the SCI is high, but the SPI is low, there could be communication issues – the evidence shows safety is being managed well, but people just don’t see it that way.

The Safety Culture Survey uses responses to calculate scores across a number of dimensions, plus an overall score.

Safety Culture Index (SCI) A factual measure of how safely a production is operating in practice. It’s based on real evidence – such as completed checks, incident reports, and observed behaviours – rather than opinions.
Safety Perception Index (SPI) A measure of how people feel about safety on a production. It reflects attitudes, confidence, and trust in safety processes — how safe people believe the working environment is.

Safety Compliance Assesses how consistently legal, regulatory, and company safety requirements are met — including paperwork, permits, and mandatory controls.
Safety Culture Looks at the shared values, attitudes, and behaviours that define how seriously safety is taken day to day. A strong safety culture means everyone looks out for each other.
Planning & Proactivity Measures how well risks are anticipated and controlled before they cause problems. It reflects preparation, foresight, and learning from past experiences.
Communication Evaluates how clearly and effectively safety information is shared across departments and teams — from toolbox talks to production meetings.
Accountability Examines whether people at all levels take ownership of safety performance, follow through on actions, and accept responsibility for outcomes.
Resourcing Considers whether there are enough people, time, and budget to carry out tasks safely — ensuring corners aren’t cut due to pressure or constraints.
Contractor Management Assesses how well safety standards are communicated, monitored, and enforced for external suppliers, freelancers, and contractors working on the production.
You can export all entries into a spreadsheet for analysis. If you don’t need all the columns, use the filter.

Anyone working in the industry can fill out a survey about their current / last production. It’s anonymous. You don’t even need to log in. All the responses are collated below and can be downloaded for analysis.

If you want to send out a survey, you can fill in basic details here (type of production, location, duration etc) and you’ll be sent a special link and QR code by email, along with a unique ID code.

You can forward this link to others, and when they use it the basic details you filled in will be pre-populated in the survey form (but not any details that could identify the production, event or company) along with the unique ID. This ID appears in the database below so you can find surveys you have initiated.

You can also opt to have responses sent to you by email – but these will be anonymous. The form does not collect any personal data or store any commecially-sensitive data.

Safety: The Data

The Safety Culture Index (SCI) uses observable, measurable facts to score actual, lived safety behaviours on productions. The Safety Perception Index (SPI) measures how safe people feel on productions, and how it compares with others they've worked on.
Tip: use the filters in the table footer or the Search box to focus on a specific production, genre, or category. The averages and the chart will update automatically.
